Abstract: Es wird das WebTed-System für webbasierte Telediagnostik vorgestellt. Es unterstützt drei Aspekte der Telepathologie: statische, dynamische und quantitative Telepathologie. Statische Telepathologie wird für Expertenkonsultationen eingesetzt. Mit einem Webbrowser werden klinische Daten und Bilder zu einem Fall durch einen Referenten auf einem Server abgelegt. Ein auf dem Server abgelegter Fall wird entweder durch einen einzelnen Konsultanten befundet oder durch mehrere Experten gemeinsam in einer Konferenzsitzung. Nachteil dieser Telepathologie ist die feste Vorgabe der Ausschnitte eines Präparats. Diesen Nachteil behebt die dynamische Telepathologie. Der Konsultant bedient ein ferngesteuertes Mikroskop und hat selber die Kontrolle über die Bildselektion. Dabei lassen sich alle wesentlichen Funktionen des Mikroskops wie z.B. Fokus, Objektiv und Licht ferngesteuert bedienen. Zweck der quantitativen Telediagnostik ist die Bestimmung von Parametern zur Beschreibung von Gewebestrukturen wie z.B. der Zellmorphologie (Form, Struktur, Verteilung, Häufigkeit) oder molekularbiologischen Parametern (Aktivität des H19 Gens), die zu einer Objektivierung der Diagnose beitragen: Die Bestimmung dieser Parameter wird vollautomatisch vom Computer durchgeführt und ist deshalb nicht durch die Subjektivität eines Pathologen beeinflußbar. Als Client wird ein vom Webserver heruntergeladenes Java Applet verwendet, mit dem man Zugriff auf die statische, dynamische und quantitative Telepathologie hat. Auf dem Server wird eine SQL-Datenbank zur Speicherung von Daten zu Bildern und Fällen eingesetzt, ein Konferenzserver koordiniert den Datenaustausch zwischen den Clients während der Onlinekonsultationen, ein CORBA ORB stellt ein Modul zur Fernsteuerung eines Mikroskops für die dynamische Telepathologie bereit, und die Bildverarbeitungskomponente wird für die quantitative Analyse der Bilddaten im Rahmen der quantitativen Telepathologie eingesetzt.

Übersetzung des Abstracts: The WebTed system for web-based tele diagnostics is presented. It supports three aspects of the tele pathology: static, dynamic and quantitative tele pathology. Static tele pathology is used for expert consultations. With a web-browser clinical data and pictures belonging to a case are stored by a refering physician on a server. A case stored on the server can either be diagnosed by individual consulant or by several consultants together in a conference session. Disadvantage of the static tele pathology is the fixed selection of a view of the specimen. The dynamic tele pathology avoids this disadvantage. The consultant uses a remote controlled microscope. All substantial functions of the microscope like focus, objective and light can be remote controlled. Purpose of the quantitative tele pathology is the determination of parameters for description of tissue structures like cell morphology (form, structure, distribution, frequency) or molecular biological parameters (activity of the H19 gene) in order to make a diagnosis more objective: The determination of these parameters is executed by the computer and is therefore not influenceable by the subjectivity of a pathologist. As client for the WebTed system a Java applet is used, with which the user has access to the components for static, dynamic and quantitative tele pathology. On the server a SQL data base is used for storage of pictures and clinical information, a conference server coordinates data exchange between the clients during the on-line consultations, a CORBA ORB supplies a module for the remote controls of a microscope for the dynamic tele pathology and the image processing component for the quantitative analysis of the visual programming environment for the quantitative tele pathology is used.
